How to Request Shoes

We are so happy for the opportunity to provide 

shoes to those in need! 

However, we are asking all recipients to make a 

pledge upon shoe delivery to engage in 

physical activity 30 minutes a day, 5 days a week. 

This activity time should be in addition to regularly 

scheduled recess and PE.  Registration 

and participation with 

Healthy Kids Club and Healthy Kids Run Series is also required. 

 

To begin the program and request shoes, click below to fill out the shoe requisition form, then click on the tab ' Kids Running Club'  in the left column on this page and  fill out the required forms.
